I understand that vaccine records are necessary for grooming services to be performed. By signing I agree to have information verbally released, in written form, or emailed to grooming by Jessica. In case of emergency if I am not able to be contacted, Grooming by Jessica staff is allowed to seek medical treatment for my pet at my expense. I agree to pick up my pet within 30 minutes of receiving notice that my pet’s grooming is completed unless otherwise arranged.
